4-H HORSE SHOW ENTRY FORM
2010 SANDWICH FAIR
Registration Deadline: Entries must be postmarked by September 15th. No entries will be accepted the day of the show.

- ENTRY FEE $15.00 ~ Payment may be made the day of the show.
- Make checks payable to CARROLL COUNTY 4-H HORSE PROJECT.
Warm up time in the ring will be given before the start of the first Equitation class in each division.
NO RIDING will be allowed in the ring before Fitting and Showmanship classes.
NOTE: The State of New Hampshire may be randomly appearing and selecting animals to test.

Pursuant to R.S.A. 508:19, equine professionals are not liable for damages resulting from inherent risks of equine activities.
4-H Exhibitor Entry Statement
I understand that riding horses can be a dangerous activity. Therefore, I agree to hold blameless for any accidents that may occur at this activity the organizers and sponsors of the activity, including but not limited to the Sandwich Fair Association, and the Sandwich Fair Horse Show Committee. I further agree to abide by the rules for the show and by all policies and decisions of the Sandwich Fair Horse Show Committee.
I have read and understand, consent to, and agree to abide by the Guidelines for 4-H Shows as stated in the 4-H Livestock Department guidelines. A separate copy may be obtained on request from the Carroll County Extension Office.
I have read and understand, consent to and agree that all 4-H animals are shown at the risk of the 4-H owner or 4-H lessee. Any damage to persons or property are the legal and financial responsibility of the 4-H member who owns or leases the animal and their family
